Illness: Illness may be acute or chronic. For a significant, acute illness (such as the flu, pneumonia, kidney infection) for which your child is out of school for longer than 3 days, please notify the nurse. A doctor's note will be required for extended absences. If your child is going to be out and you would like to receive make up work/homework, his/her guidance counselor should be notified as well. For chronic illnesses ( ie. epigastric reflux disease, asthma, diabetes, depression, anxiety, seizures, allergies, etc) the nurse should be notified at the beginning of school, so a plan of care can be developed for your child. If your child has any of the conditions listed below, please fill out the appropriate carepan with MD signature as required and return to the nurse's office.

Food Allergies: If your child has severe food allergies requiring an epi-pen, you must fill out the Severe Allergy Careplan in the table above and return it to the nurse's office. We have a severe allergy protocol, where these students are indentified and most of the school staff is trained to administer an Epi-pen in an emergency. We require a trained chaperone on all field trips if there is a student with an Epi-Pen attending. Nuts are discouraged in the classrooms, please call the school nurse if you have specific questions. There is a nut-free table available in the cafeteria. The cafeteria food is considered "nut-free" as they don't purchase or prepare any products with nuts and they monitor traces of nuts, but can not guarantee "inadvertent" nuts (caused by cross contamination). See Whitson's position statement for more information. For other allergies, not requiring an epi-pen, it is recommended that you let the nurse know as well. The cafeteria staff is given a list of students with allergies at the begining of the year that they enter into their computer system. Ingredient lists are available on all foods served by the food staff. If students have any questions regarding what they can eat, they should ask one of the servers who will obtain that information for them. If you have particiular questions or concerns, please contact the Food Services Director, Scott Berry directly at ext. 6140.

Injury: If your child has an injury that results in decreased mobility or movement, please notify the nurse. The nurse will make the necessary accomodations (ie use of the elevator, evacuation plan, ensure notetaking capability, extra set of books, etc). For accomodations to be in place for longer than a day or two, a Doctor's note is necessary.

Medical Excuse from Physical Education: if one to three gym classes will be missed, a parent note must be given to the physical education teacher or nurse. If more than three gym classes will be missed a physician's note is required. The nurse reviews all physical education medical excuses at the end of each quarter.
