The following immunizations are required for school attendance:

• 3 doses of Hepatitis B OR 2 doses of Merck's Recombivax HB
• 4 doses DTaP/DTP OR 3 or more doses of Td; plus 1 Td booster
• 2nd doses measles, mumps, rubella
• 1 dose of varicella vaccine if less than 13 yrs. old; 2 doses of varicella vaccine if 13 yrs. of age or greater OR a physician certified history of chicken pox

Please send completed Immunization form to the nurse's office prior to the start of schoo. The only exception to the above is an exemption based on medical reasons or religious beliefs. In order to do this, the Request for Exemption of Immunizations form must be completed and returned to the nurse's office.

Failure to provide the school nurse with documentation of these immunizations prior to the start of school is cause for exclusion from school. A copy of your child's immunization record will be sent to you upon graduation. It is suggested that you keep this record or a copy in your family health file for future reference.
