Physical Examinations: Physical examinations are required for all new students, 8th graders and for athletic team participation. A copy of this physical must be on file with the school nurse prior to participation. It is recommended that the primary care physician conduct this physical, however, the school doctor is usually scheduled for physical examinations prior to or at the start of each sport season, provided there are a sufficient number of students who have made appointments with the nurse. Parents may call to make an appointment for their childInterval Health History: During your child's start in middle school (7th grade) and their junior year, we recomend completing an interval health history in order to provide us with current information regarding your child's health status. This form can be completed by a parent or guardian and does not require a physician's signature.
Immunizations: The following immunizations are required for school attendance and must be received prior to starting classes:
3 doses of Hepatitis B OR 2 doses of Merck's Recombivax HB
4 doses DTaP/DTP OR 3 or more doses of Td; plus 1 Td booster
2nd doses measles, mumps, rubella
1 dose of varicella vaccine if less than 13 yrs. old; 2 doses of varicella vaccine if 13 years of age or greater OR a physician certified history of chicken poxFailure to provide the school nurse with documentation of these immunizations prior to the start of school is cause for exclusion from school. A copy of your child's immunization record will be sent to you upon graduation. It is suggested that you keep this record or a copy in your family health file for future reference.
