Physical Examinations: Physical examinations are required for all new students, 8th graders and for participation on any athletic team. A copy of this physical must be on file with the school nurse. In order to participate in sports, the physical exam must be within 13 months of the first day of try-outs. There are no exceptions. It is recommended that the student's primary care physician conduct this physical, however, information regarding sports physicals by the school doctor will be posted prior to each season. If you need more information, please call the health office.Interval Health History: During your child's start in middle school (7th grade) and their junior year, we recomend completing an interval health history in order to provide us with current information regarding your child's health status. This form can be completed by a parent or guardian and does not require a physician's signature.
Required Immunizations: The following immunizations are required by the Massachusetts Department of Public Health for school attendance and must be received prior to starting classes:
3 doses of Hepatitis B OR 2 doses of Merck's Recombivax HB
4 doses DTaP/DTP OR 3 or more doses of Td; plus 1 Td booster
2 doses of measles, mumps, rubella
1 dose of varicella vaccine if less than 13 yrs. old; 2 doses of varicella vaccine if 13 years of age or greater OR a physician certified history of chicken poxFailure to provide the school nurse with documentation of these immunizations is cause for exclusion from school. A copy of your child's immunization record will be sent to you upon graduation. Most colleges will require this information prior to allowing the student to attend classes. It is suggested that you keep this record or a copy of it, in your family health file for future reference.
Recommended Immunizations: The following immunizations are recommended for all school aged children by the Center for Disease Control (CDC) located in Atlanta, Georgia:
3 doses of HPV (Human Papillomavirus) for all females beginning at age 11-12
1 dose of Meningitis vaccine at age 11-12
Include booster for Pertussis in required Td booster at age 11-12.
Flu vaccine for all schoolaged children
