Subject: ACTION REQUIRED: DC medical forms
I hope your summer is off to an awesome start! I am sending email updates to the email given when you signed up your student. If there is another email you wish to add, please let me know.
Along with the school’s permission slip, the other piece of paperwork needed for the DC trip is the medical form. (Google doc link to medical form)
EVERYONE must have a signed medical form to attend the trip. I have attached everything you need to this email. The forms are also available on the DC trip page (Masconomet Middle School – Extra-curricular tab).
Please carefully read the Emergency Treatment Form and pay particular attention to the information about medications. A Field Trip Nurse will be administering all medication to students on this field trip with the exception of insulin, inhalers, epinephrine auto-injectors, and digestive enzymes. No student should be in possession of a controlled stimulant.
Everyone must fill in pages 1 and 3. Page 1 is for basic medical information and emergency contacts. Page 3 is medication consent and emergency medical treatment authorization. (Please remember to fill in your child’s name and sign the bottom of page 3.)
Page 2 deals with medication. If your child is not taking any specific medication, please look at the bottom of page 2 for over-the-counter medicine that does NOT require a doctor’s order, but DOES require parental permission to give.
ANY medication, prescription or over-the-counter, requires a doctor’s order AND the administration plan (with the exception of the list at the bottom of page 2). The deadline is in September so I want to give you as much time as possible to get the forms signed, if needed.
Once the forms are filled out and signed, you may scan them or drop them off at the MS office.
Deadline for ALL medication forms is September 22!

Subject: DC participants: ACTION NEEDED
Hello DC Participants,
We leave for the Washington trip within the first 2 months of school in the fall so I would like to get ahead on the paperwork needed for the trip!
Attached please find a pdf of the permission slip for overnight field trips. (Click here for the Google docs version of the permission slip.) There will also be a copy linked on the DC webpage (found in the Masco MS home page – “Extracurricular Activities” tab).
This form must be filled out and returned by September 8. Please fill in your child’s name on the front page and sign and date the second page.
You can download, sign and scan it back to me at firstname.lastname@example.org or drop off the signed form to the MS main office this week or over the summer.
The main office will collect the forms but will not be able to answer any questions regarding the trip.
Please email me any questions or concerns you may have.

At the parent meeting, a question was asked about if CFAR insurance was taken (which gives 75% back or any reason), will it also cover the Deluxe insurance (which gives 100% back for COVERED reasons).
NOTE: The Deluxe insurance requires documentation but CFAR does not.
The answer from Gerber Travel is Yes the CFAR insurance also comes with Deluxe coverage.
This means that if you take out the CFAR insurance and your child ends up not attending due to a "covered" reason, you will get more of your money returned once you provide documentation of the covered reason.
There is financial help available, both through the company and the school.
The scholarship form is posted below the calendar.

Registration is now open! It will remain open until April 30.
Instructions on how to register will be sent by the school via email and is posted below.
Insurance MUST be taken at the time of registration. You CANNOT go back and add it once registered.
Please carefully read and fill out all 3 pages.
NOTE: If your child will be taking any medication (prescription OR over-the-counter), you will also need to fill out 2 other forms (doctor's orders and administration plan) with the exception of the medications listed at the bottom of page 2.
