Medication Administration Plan (signed by PARENT)
Along with the doctor's authorization, a parent/guardian must fill out this Administration Plan for EACH medication needed on the trip.

Medication Order Form (to be filled in by licensed physician)
Fill out ONLY if medication is needed
Use this form for EVERY medication (prescription AND over-the-counter) needed on the field trip.
(NOTE: You may use this form or a form from your doctor's office.)

Medical Form and Medication Information
EVERYONE needs to fill out the "EMERGENCY MEDICAL TREATMENT FORM" side of the form.
Fill in the backside of the form ("Prescription and Over-The-Counter Medication") ONLY if taking medication on the trip.
