Proof of Residency Requests for a Learner's Permit
To get a learner's permit, driver’s license, or Mass ID in Massachusetts, you’ll need to provide proof of Massachusetts residency from the school you are reigistered with. The Registrar can provide you with an official high school transcript and proof of residency letter in a sealed envelope.
The process is as follows:
1. Request a transcript by completing the release form found here:

2. There is a fee of $7.00 per transcript to be paid via check. Please make the check out to Masconomet Regional High School.
3. You can mail the form and check to Masconomet's main address or drop off the items at the Central Office drop box located outside their main door. Note to the attention of Kristy Jennings on the envelope.
4. Once both items are received we can process the transcript request. This document will be mailed to your home address in a sealed envelope that can then be presented to RMV.
Please allow 2 weeks for processing during the COVID-19 pandemic.

Work Permit Process During COVID-19
In order to receive a work permit the first step is to complete a permit application once a job offer has been obtained.
The application can be accessed below.
Once both pages are completed email the application to the high school Registrar, Kristy Jennings, at kjennings@masconomet.org
The second page of the permit will then be emailed to you for the signature of the student.
Once signed email that form back to Kristy Jennings so the rest of the permit to can be released to you via email.
At that point you can pass in the permit to your new employer and will be all set.
All teens under 18 years of age must complete a work permit application and get a work permit before starting a new job.
For minors who are residents of the Commonwealth of Massachusetts, permits are issued by the superintendent of schools for the municipality in which the minor lives or attends school. If the minor lives outside the Commonwealth, the permit is issued by the superintendent for the community where the minor's job will be located.
Minors must provide the specific employer, work address, and job description to receive a permit.
Minors who are not current students of Masconomet Regional School District but live in the towns of Boxford, Middleton and Topsfield can receive a worker permit from our Guidance department in the High School. The minor must come with the completed form, birth certificate, and Photo ID (if applicable).

Current Student Transcript Requests
All current Masconomet High School students may request a Transcript using the Record Release Request form. There is a fee associated with each Transcript requested.
- Unofficial Transcripts cost $2 each
- Official Transcripts cost $7 each
The check can be made out to: Masconomet Regional High School
The completed form and check can be returned to the Registrar in the High School.

Senior Forms
Permission for Release of Records - "Purple Form"
Seniors who need to send their High School transcripts to Colleges via Naviance must complete the form below and submit to either their Counselor or the Registrar by the disgnated deadline. Once received the authorization will be granted in Naviance for the student to access this information.

Scholarship Request/Release Form
This form is primarily used by Seniors to request Official Transcripts and a Guidance Letter of Recommendation for Scholarships they are applying for. Once completed the form and payment can be returned to the Registrar.
In order to complete the form, please download it first, then complete electronically. It is a fillable form.

NCAA Eligibility Information
If you want to play sports at a NCAA Division I or II school, start by registering for a Certification Account with the NCAA Eligibility Center.
If you want to play Division III sports or you are not sure where you want to compete, start by creating a Profile Page.

Student Withdrawal
A student indicating that they will be leaving school should be referred to their Conunselor who will ask for parental contact first.
- The student must have the parent/gaurdian signature to begin this process.
- All Teachers listed on thier current schedule would collect items back and sign off on the form.
- The Homeroom Teacher would make sure the locker has been cleaned out and left satisfactory, then would sign off on the form.
- The Counselor would approve the form and sign off on it.
- A photocopy of the completed form with an "Unofficial" Stamp and an "Unofficial" Transcript would go home with the student. The original would stay within the Guidance records.

Alumni Transcript Requests
All Masconomet High School Alumni may request a Transcript using this form below. There is a fee associated with each Transcript requested.
- Unofficial Transcripts cost $2 each
- Official Transcripts cost $7 each
The check can be made out to: Masconomet Regional High School
Allow 5 business days for processing once the form and payment have been received. Mail both items to the attention of the High School Registar.
