Proof of Residency Requests for a Learner's Permit
To get a learner's permit, driver’s license, or Mass ID in Massachusetts, you’ll need to provide proof of Massachusetts residency from the school you are enrolled at. The Registrar can provide you with an official high school transcript and/or proof of residency letter in a sealed envelope.
The process is as follows:
1. Request a transcript by completing the release form found here: Records Release Request Form
2. There is a fee of $7.00 per transcript to be paid via check. Please make the check out to Masconomet Regional High School.
3. The form and check can be dropped off at the School Counseling Office or placed in the dropbox located at the main entrance of the high school - to the right of the doors. Note to the attention of Kristy Jennings on the envelope.
4. Once both items are received, we can process the transcript request. This document can be picked up by the student during the school day. It will be in a sealed envelope that can then be presented to RMV.

Work Permit Requests
Obtaining A Work Permit
Minors (Ages 14-17)
In order to receive a work permit, the first step is to complete a permit application once a job offer has been obtained.
The application can be accessed below.
Once both pages are completed, the Masco student can drop off the application to the high school Registrar, Kristy Jennings, within the School Counseling Office or email it to kjennings@masconomet.org
The Masco student will then be prompted to come back to pick up the completed permit.
If the student lives in the tri-town district but attends a different high school, this process can be done through email with Kristy Jennings, kjennings@masconomet.org
Email the 2-page application along with a copy of the student's birth certificate or passport. In addition, please include this information in the initial email:
1. Hair Color
2. Eye Color
3. Facial Marks
4. Current School
5. Last Grade Completed

Alumni Transcript Requests
All Masconomet High School Alumni may request a transcript using this form below. There is a fee associated with each transcript requested.
- Unofficial Transcripts cost $2 each
- Official Transcripts cost $7 each
The check can be made out to Masconomet Regional High School. It can be mailed or placed in the dropbox located at the entry of the high school, door #32. The dropbox is accessible 24/7 and checked multiple times a day.
Allow 5 business days for processing once the form and payment have been received. Mail both items to the attention of the High School Registrar.

Senior Forms
Scholarship Request/Release Form
This form is primarily used by Seniors to request Official Transcripts and a School Counseling Letter of Recommendation for Scholarships they are applying for. Once, the form and payment can be returned to the Registrar, Mrs. Jennings, within the School Counseling Office.
In order to complete the form, please download it first, then complete it electronically. It is a fillable form.

Student Withdrawal
A student indicating that they will be leaving school should be referred to their School Counselor who will ask for parental contact first.
- The student must have the parent/guardian's signature to begin this process.
- All teachers listed on their current schedule would collect items back and sign off on the form.
- The Homeroom Teacher would make sure the locker has been cleaned out and left satisfactory, then would sign off on the form.
- The School Counselor would approve the form and sign off on it.
- A photocopy of the completed form with an "Unofficial" Stamp and an "Unofficial" Transcript would go home with the student. The original would stay within the School Counseling records.
