MASCONOMET REGIONAL SCHOOL DISTRICT HEALTH SERVICES INFORMATION

The Masconomet nurses would like to bring parents and students up to date on important information for the returning to school.

  

NEW STUDENTS:  Medical records from the student’s previous school must be presented to the school nurse prior to entry.  Up to date immunizations and documentation of Tuberculosis Risk Assessment (both as outlined below), as well as a physical examination within one year of entrance are required before your student attends classes.  Your child may be excluded from school if you do not provide this documentation.  

 

SPORTS PHYSICALS:  Any student playing a fall sport must have a physical examination within 13 months prior to the start of try-outs.  In order to participate in practice, documentation of the physical must be in the school 24 hours prior to the start of try-outs in August. Try-out dates can be found on the Masconomet website.  This is a ruling of the MIAA (Massachusetts Interscholastic Athletic Association) Sports Medical Committee and there will be NO EXCEPTIONS.

 

MEDICATIONS:  All medications (short-term or long-term) must be in a pharmacy or manufacturer-labeled container with appropriate forms filled out (including physician signature). These forms are available on the Health Services website for your convenience.  A parent, guardian or parent/guardian designated adult is responsible for delivering all medications to the school nurse.  No more than a thirty (30) day supply of the medication shall be stored at the school.  No student is allowed to carry any medication on their person with the exception of insulin, asthma inhalers, pancreatic enzymes and Epinephrine auto-injectors.

 

STUDENTS WITH ASTHMA, SEVERE ALLERGIES OR OTHER CHRONIC HEALTH CONDITIONS:  Please be sure to return any forms prior to the start of school that you received over the summer regarding your child’s asthma, allergies, diabetes or seizures. Be sure to include the signature of your child’s primary care provider.  This will allow us to provide your child with appropriate care during the school day. Please be aware that without this information, your child may not be able to attend school field trips. These forms are also available on our website.



IMMUNIZATION REQUIREMENTS: 

All Students Grade 7– 12:

Dtap/DTP             4 doses DTP/DTaP or 3 doses of Td; plus 1 dose of Tdap 

POLIO                  ≥ 3 doses

MMR                    2 doses 

HEPATITIS B       3 doses                                 

VARICELLA         2 doses or a physician documented history of the disease

MenACWY           1 dose for all incoming 7th and 2 doses for 11th grade students

 

Students in all grades must also have documentation of a Tuberculosis Risk Assessment (not an actual TB test) done by their PCP and usually documented on your child’s physical.
